Confused at how much I'll be paying

Hey, this is the setup I am trying to achieve on my iPhone.

-Buy Italian local # for 2.95$/mo
-Connect via Gtalk. Have people in italy call that # resulting in free on their end, free on my end. (I guess the Gtalk app rings and calls are through that?)
-When I want to call, I open Gtalk. I dial their number and it is free on my end, free on their end. (Are outgoing calls included, also to italian mobile?)

If this is the setup, sign me up!

Yes , as long as you have proof of address in the Italy city you want a number

Yes, its local for them and free for you to receive

You would be using Gtalk infrastructure to make outgoing calls, we have nothing to do with that.

You are better off using SIP and a SIP client to make and receive calls. There is a low per min rate for outgoing calls and this is the only way you can have the FlyNumber show up as the caller- ID
